Since quite a few people have asked me how to do this, and I keep losing the information on this I thought I would just post it here that way from now on everyone (including myself) can just reference it here.
First I went to 123 Contact Form.
Click Sign up for FREE
Fill in the information to sign up and then Click Create Account.
Now you should see something that says You Have No Forms, Create One Now-Click on that
You will Name your form. (Contact Tab or whatever you choose to name it.)
You will see the Form Editor and it is showing you what will be under your contact tab.
I just leave it with the Name, Email, Subject and Message, but if you want to add something else you can do so below where it says Add Field Name.
Click NEXT
You can change the color if you would like and then click NEXT
Click SAVE
Go to Embed form in a page hosted on your server and paste that.
Now here is how to add it to your Facebook page…
Before you can use the code on Facebook you need to set up a Static FBML tab. You can do this by going to your Facebook page, then “Edit Page” (It’s right below your profile picture.) Scroll down to the bottom of the page, go to More Applications and you should see a Static FBML option there.
(This time when I did it I actually had to type in Static FBML in the Search box to get it to bring me to the page where I could do the next step.)
Select Add to My Page (right below the Question Mark-profile pic.)
If you already have the Static FBML tab you will just select edit for the FBML application and then scroll down to the bottom of the page and select Add Another FBML box.
When you have done that you go back to your page and select Edit Page again.
Select Edit (under the FBML Application.)
From there you type in the name/title you want for the Tab (I just put Contact, you could put Email or whatever you would like there.) After you have done this you will need to copy and paste the code that you created when you were at 123ContactForms.
Save!
